Appointments
We schedule appointments Monday through Friday, 8:00 a.m. to 5:30 p.m. Surgery and
test scheduling can be done in our office.
Initial Visit
On your initial visit, please bring:
Updated Insurance card
Referral( if required by your insurance company)
List of allergies
List of current medications (prescription or over-the-counter) including name, dosage, frequency
Related x-rays.
Records from the referring physician.
Payment and Insurance
Payment is due at the time of service. For your convenience, we accept Visa,
MasterCard, or debit card. If your visit is covered by your health insurance, we will file
a claim with your insurance company. AUI is a participating provider for Blue Cross
and Aetna. We also accept Medicaid and Medicare. If you are covered under Worker's
Compensation and they do not honor the charges, you are solely responsible for the
account balance within thirty days.
Drug and Device Studies
We participate in many research studies of new drugs, devices, and indications.
These studies benefits our patients by giving them access to new cancer agents, new
therapies such as the oral pills for sexual dysfunction, and therapies that they might
not afford. For more information on current studies, please visit the Alaska Clinical
